29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: On or about March 9, 2023, an employee was exposed to fall hazards while working from a supported scaffold platform, supported by a skid steer equipped with forks and being used as a forklift and the employer did not ensure that the platform was attached to the forks.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: On or about March 9, 2023 an employee was exposed to fall hazards while working from a scaffold platform supported by the forks of a powered industrial vehicle (PIV) and the employer did not ensure that the employee was protected from the fall hazards by the use of a complete guardrail system or person fall arrest system (PFAS).
