29 CFR  1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel.  a) 200 River Rd. Building 3 - On or about February 10, 2023 an employee was standing on the forklift forks elevated approximately 8 feet above the surface to access a mezzanine without a safety cage and platform.   b) 200 River Rd.
