29 CFR  1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 03/09/2023, in Mill 1 shop; employees were exposed to amputation hazards when lockout/tagout procedures were not utilized to perform servicing and/maintenance procedures such as but not limited to; replacing belts and lubricating the Timesave2300-Cantek 26? Planer.  b) On or about 03/09/2023, in Mill 4 shop; employees were exposed to amputation hazards when lockout/tagout procedures were not utilized to perform servicing and/maintenance procedures such as but not limited to; replacing blades, cleaning, and setting up the Brandt Homag KDF 670 Banding Machine.  c) On or about 03/09/2023, in CNC room; employees were exposed to amputation hazards when lockout/tagout procedures were not utilized to perform servicing and/maintenance procedures such as but not limited to; cleaning and/or changing filters and oiling the Cosmec Conquest 515 CNC machine.  ABATEMENT CERTIFICATION REQUIRED

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:   a) On or about 03/09/2023, throughout the establishment; Employer did not provide Lockout/Tagout (LOTO) training to authorized employees. Employees performed maintenance and servicing activities on machines such as, but not limited to: replacing belts on the Timesave2300-Cantek 26? Planer, replacing blades on the Brandt Homag KDF 670 Banding Machine, and cleaning/replacing filters on the Cosmec Conquest 515 CNC machine. Employees were not trained as LOTO authorized employees.  ABATEMENT DOCUMENTATION REQUIRED

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  a) On or about 03/09/2023, throughout the establishment; where employees perform servicing and maintenance on machines and equipment such as but not limited to; replacing belts on the Timesave2300-Cantek 26? Planer, replacing blades on the Brandt Homag KDF 670 Banding Machine, and cleaning/replacing filters on the Cosmec Conquest 515 CNC machine.  The employer did not review the energy control program and procedures and each employee's responsibilities under the energy control procedure being inspected, at least annually.    ABATEMENT CERTIFICATION REQUIRED
