29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Re-roofing commercial structure located at 726 Jesse Jewell Pkwy, Gainesville, GA 30501, Ground level: On or about 3/07/2023, and at times prior, the employer exposed employees to struck-by hazards while operating a TranzSporter TP250, in that protective helmets were not provided when lifting material and equipment from the ground area to the roofing working area above.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Re-roofing commercial structure located at 726 Jesse Jewell Pkwy, Gainesville, GA 30501, Ground level: On or about 3/07/2023, and at times prior, the employer exposed employees to fall hazards of approximately 21 feet to the lower ground level, in that adequate fall protection system was not provided to employees engaged in shingle removal activities

29 CFR 1926.552(a)(1):29 CFR 1926.552(a)(1): The employer did not comply with the manufacturer's specifications and limitations applicable to the operation of all hoists and elevators. a) Re-roofing commercial structure located at 726 Jesse Jewell Pkwy, Gainesville, GA 30501, Ground level: On or about 3/07/2023, and at times prior, the employer exposed employees to struck-by hazards while operating a TranzSporter TP250 , in that the manufactured installed wire rope was replaced by a nylon climbing rope, in which directly contradicts the manufactures recommendations for a replacement cable.
