29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) At the jobsite located at 10726 Osprey Landing Way, Thonotosassa, FL 33592; on or about 03/10/2023, the employer exposed employees to fall hazards in that fall protection was not utilized while roofing and framing operations were being conducted on a residential home approximately 15 feet in height.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) At the jobsite located at 10726 Osprey Landing Way, Thonotosassa, FL 33592; on or about 03/10/2023, the employer did not provide adequate training for employees conducting framing operations where employees were exposed to 15 foot fall hazards, in that, fall protection training was not provided.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.) At 10726 Osprey Landing Way, Thonotosassa, FL 33592, on or about 02/14/2023:  employees accessed a roof  approximately  15 feet from the ground below from a extension ladder whose side rails extended only about 14 inches above the roof and upper landing surface.
