On or about June 6, 2023, the employer failed to submit abatement documentation and Certification of Corrective Action for citation 1 item 1, issued on April 28, 2023, with an abatement due date of May 4, 2023.  On or about June 6, 2023, the employer failed to submit abatement documentation and Certification of Corrective Action for citation 1 item 2, issued on April 28, 2023, with an abatement due date of May 4, 2023.  On or about June 6, 2023, the employer failed to submit abatement documentation and Certification of Corrective Action for citation 1 item 3, issued on April 28, 2023, with an abatement due date of May 4, 2023. 29 CFR  1926.451(g)(4)(i): Guardrail systems was not installed along all open sides and ends of platforms. Guardrail systems was not installed before the scaffold is released for use by employees other than erection/dismantling crews.   On or about March 13, 2023, employer did not install a guard rail system before employee started working on the scaffold . This condition exposed employee to falls from above 10 feet to ground level.
