On or about May 26, 2023, the employer failed to submit documentation and certification of corrective action for citation 1 item 1, issued on March 23, 2023 with an abatement due date of April 14, 2023.  On or about May 26, 2023, the employer failed to submit documentation and certification of corrective action for citation 1 item 2, issued on March 23, 2023 with an abatement due date of April 14, 2023.  On or about May 26, 2023, the employer failed to submit documentation and certification of corrective action for citation 1 item 3, issued on March 23, 2023 with an abatement due date of April 14, 2023.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 14, 2023, employees engaged in residential construction activities were utilizing a multi-position ladder that did not extend at least 3 feet above the upper landing surface. This exposed employees to fall hazards.
