29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):  (a) 23 Washington Ave, Golden, CO 80403: On or about March 15, 2023, and at times prior, the employer did not provide a protective system to protect employees from cave-in hazards while working in an excavation in excess of 7.5 feet deep. Employees were exposed to cave-in hazards when measuring and marking elevation in the excavation. Walls of the excavation were not sloped or benched adequately, and no shoring or shielding was provided for use.
