29 CFR  1926.453(b)(2)(iv): The employer did not ensure that employees always stood firmly on the floor of the basket, and did not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position:   a) Job site, 7507 McKnight Road, Pittsburgh, PA: On or about March 15, 2023, during demolition activities, the employer did not ensure that employees remained on the basket floor in that employees climbed up and over the basket guardrails to get in/out of the the basket while at the top of the building, exposing them to falls of up to approximately 25 feet.

29 CFR  1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift:    a) Job site, 7507 McKnight Road, Pittsburgh, PA: On or about March 15, 2023, during demolition activities, the employer did not ensure that employees utilized appropriate fall protection systems/devices while working in aerial lifts where exposed to fall hazards of up to approximately 25 feet.    Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR  1926.501(b)(1):  Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) Job site, 7507 McKnight Road, Pittsburgh, PA: On or about March 15, 2023, during demolition activities, the employer did not ensure that employees utilized appropriate fall protection systems/devices while working on the roof of the building where exposed to fall hazards of up to approximately 25 feet.
