On or about March 16, 2023 during the maintenance of a roof at a job site located at 1661 OH 162, Akron Ohio 44320; during the maintenance of a roof where employees had to gain access/egress, the employer did not ensure portable ladders extended 3 feet above the upper landing surface.

29 CFR  1910.28(b)(1)(i):29 CFR 1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  On or about March 16, 2023 during the maintenance of a roof at the job site located at 1661 OH 162, Akron Ohio 44320; the employer did not ensure employees working at a height of approximately 11.6 feet above the ground were protected from a fall hazard by utilizing personal protective equipment.
