29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a) South Side of Building - An employee was required to access a commercial roof with an aerial lift and was exposed to a fall hazard of approximately forty-two (42) feet because he was not wearing a body belt with a lanyard attached to the basket, on or about March 20, 2023. 29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  a) South Side of Building - Employees were required to ride in an aerial lift to access a commercial roof approximately forty-two (42) feet high without first being trained by a qualified person on the hazards associated with working from, and riding on, the aerial lift, on or about March 20, 2023.  Abatement Certification Required.
