1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  Meat Receiving Bay, 94 Foster Road, Southwick, MA: On or about March 22, 2023, and at times prior, the employer did not verify in writing that a workplace hazard assessment had been performed to ensure employees were protected from thermal burn hazards when performing work in the vicinity of the truck tank steam /hot water cleaner. Hot steam/hot water supply pipes and vented steam/hot water in the uncovered condensation tank were located next to a worktable where PPE was stored as well as the meat receiving bay Lock Tag out center.

29 CFR  1910.29(f)(4): Openings in a stair rail system exceeded 19 inches at its least dimension:  94 Foster Road, Southwick, MA: On or about March 22, 2023 and at times prior, the stair rail systems in the following areas were not equipped with an intermediate rail resulting in an opening of approximately 30 inches between the top of the tread and bottom of the hand rail:  a) Meat receiving bay: Stairs leading to control panel missing intermediate rail. b) Meat receiving bay: Stairs leading to Upper level was missing a intermediate rail. c)  Waste oil receiving bay: Stairs leading to loading dock was missing a intermediate  rail. d)  Waste oil receiving bay: Stairs leading to upper level was missing a intermediate rail.
