29 CFR  1910.157(c)(1):The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.  a) On or about March 23, 2023, portable fire extinguishers in front of the Bonfiglioli control room Conex box were not mounted or identified to make them readily accessible to employees, thereby exposing employees to fire hazards.  b) On or about March 23, 2023, portable fire extinguishers in the scrap yard, near the Bonfiglioli grinding mill were not mounted or identified to make them readily accessible to employees, thereby exposing employees to fire hazards.  c) On or about March 23, 2023, portable fire extinguishers in the scrap yard, near the picker station Conex box were not mounted or identified to make them readily accessible to employees, thereby exposing employees to fire hazards.

29 CFR  1904.29(a):Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.  a) On or about March 23, 2023, the employer failed to maintain OSHA 300, 300-A, and 301 forms, or equivalent forms to record work-related injuries and illnesses.
