29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:  a) Permafloor LLC, jobsite located at 3050 Red Lion Road, Philadelphia, PA 19114: On or about April 12, 2023, the employer did not provide medical evaluations to employees using respirators such as but not limited to Honeywell half-face 7700 respirator and North half-face 7700 respirator while performing sanding and painting activities.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  a) Permafloor LLC, jobsite located at 3050 Red Lion Road, Philadelphia, PA 19114: On or about April 12, 2023, the employer did not verify that the required workplace hazard assessment had been performed through a written certification which includes: the identity of the workplace evaluated, the person certifying the evaluation had been performed, and the date the hazard assessment was done for employees performing sanding and painting activities.
