Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system: a) On or about March 24, 2023, employees were exposed to falls of approximately 23.3 feet or more above ground level while engaged in residential framing activities. Fall protection was not being utilized while installing rafters on a new residential construction.

29 CFR 1926.1051(a):A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. (a): On or about March 16, 2023, the employer did not ensure employees engaged in construction activities were protected from a change in elevation of approximately 21 inches. Employees were exposed to tripping and fall hazards while entering and exiting the structure through the north side.
