29 CFR 1910.67(c)(2)(iv): Employee(s) were not standing firmly on the floor of the basket: (a) Jobsite: On or about March 24, 2023, an employee had one foot hooked to the outside of the man basket while he was leaning through the top rail and the mid rail of the man basket. In addition, this employee was not tied off. The fall hazard to the employee was about 20 feet.

29 CFR 1910.67(c)(2)(v):A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: (a) Jobsite: On or about March 24, 2023, two employees were working from an aerial lift (man basket attached to an Elliott crane) and neither employee was tied off. The fall hazard to the two employees was about 20 feet to the ground.
