29 CFR 1910.272(j)(1): The employer did not develop and implement a written housekeeping program that established the frequency and the method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other expose surfaces: a) General facility: On March 28, 2023 and times prior a housekeeping program had not been implemented to manage accumulation of fugitive grain dust on ledges, floors, equipment or other exposed surfaces to reduce the hazards of combustible dust. The frequency and methods of housekeeping used were not enough to keep up to three (3) inches of fugitive, combustible dust from accumulating in areas including but not limited to, the south ground corn drag conveyor, truck loadout/scale barn, elevator pits, and in the tunnels. Note) Certification and documentation of corrective actions are required for this item.

29 CFR 1910.272(j)(4): The housekeeping program did not address the procedures for removing grain and product spills from the work area: a) Grain Handling facility: On March 28, 2023 and at times prior grain product spillage accumulated in several areas throughout the facility and was not removed for up to 3 weeks at some locations. These areas included, but were not limited to the tunnels and elevator pits. Note) Certification and documentation of corrective actions are required for this item.
