The frequency and methods of housekeeping used were not enough to keep up to three (3) inches of fugitive, combustible dust from accumulating in areas including but not limited to, the south ground corn drag conveyor, truck loadout/scale barn, elevator pits, and in the tunnels.        Note) Certification and documentation of corrective actions are required for this item.
