29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about March 28, 2023, during the installation of siding material at a job site located at 7538 Rolling Green Ave NW, Massillon, Ohio 44646, the employer did not ensure employees working on a portable ladder directly below the elevated employee was protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 17.2 feet.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  On or about March 28, 2023, during the installation of siding material at a job site located at 7538 Rolling Green Ave NW, Massillon, Ohio 44646; the employer did not ensure employees working on a ladder jack scaffold were protected from a fall of approximately 17.2 feet by utilizing a fall protection system.
