29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a)  The employer failed to submit documentation of abatement within 10 days of the abatement date for Citation 1 Item 1, Citation 1 Item 2a, Citation 1 Item 2b.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 28, 2023, employees installing shingles on the roof of a multi-level residential building, at a height greater than six feet, were not protected from falls to the lower level.

29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  On or about March 28, 2023, employee was suspended from a mobile scaffold was not protected from a fall greater than 10 feet above a lower level.  The scaffold system being utilized at the time of the inspection did not have standard guardrails on one of the ends folded up and in the fixed position.

29 CFR  1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design:   On or about March 28, 2023, an employee was suspended from a mobile scaffold that was not designed and constructed by a qualified person.    a) The scaffold did not have standard guardrails on all sides.  b) The scaffold did not contain information with the weight of the platform and the working load capacity.  c) The scaffold did not have a means to securely attach the platform to the lifting mechanism.
