Employees use a hand cart throughout the day to stock and/or retrieve product from the storage area. On or about March 29, 2023. Compressed gas cylinders including, but not limited to nitrogen and carbon dioxide were not secured to prevent it from falling.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source, and rendered inoperative: a) Grossglockner Inc dba Josef's Artisan Meats, 2640 Brickyard Road, Canandaigua, NY 14424. On or about March 29, 2023. Employees clean and change-out the blades of the Seydelmann bowl chopper daily, Monday-Thursday. The employer did not establish a program consisting of employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury. The chopper has multiple sources of energy including pneumatic, electric, and hydraulic.

On or about March 29, 2023. Authorized employees manually move, clean and change-out the blades of the Seydelmann chopper. The LOTO procedures were not clear and specific lacking the intended use of the procedure, specific shut down instructions, placement and removal rules and techniques for the control of hazardous energy and testing to determine the lockout is effective.
