(a) On or about March 22, 2023, in the Service Drive area, the employer did not ensure an employee involved in work activities that included working on and around an energized 277 (VAC) 3 phase electrical circuit as a part of a High-Bay lighting fixture replacement upgrade project, had been protected against electrical shock by proper deenergization of the effected electrical circuit at, and in the electrical control panel marked; A1 277/480 volt, located in the back of the parts room prior to the employees exposure to the electrical wiring at and for each of the approximately eight (8) High Bay florescent light fixtures removed and then replaced with a hard wired LED fixture. (a) On or about March 29, 2023, employees working at and around the base of the scissor lift used to complete tasks associated with the High Bay lighting fixture replacement upgrade project, were exposed to a struck by hazard when tools, light fixtures and associated electrical and suspension components, and boxes were piled to a height higher than the top of the toe board without adequate paneling or screening being installed.
