OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from flying chlorinated polyvinyl chloride (CPVC) pipe fragments: a) On or about March 29th, 2023, and times prior, the employer, Western Air Flight Academy, an American Flight School dba Western Air Flight Academy, LLC, did not ensure employees required to use pneumatic tools attached to an airline composed of chlorinated polyvinyl chloride (CPVC) pipe were protected from struck-by hazards. The measured air pressure in the line was approximately 105 PSI.
