29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a)  At 30 Ash Street, Santa Rosa Beach, Florida:  On March 30, 2023; the employer failed to ensure employees working from an aerial lift to install gutters on a residential house were using fall protection exposing his employees to a fall hazard of up 30 feet to the ground below.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   a)  At 30 Ash Street, Santa Rosa Beach, Florida:  On March 30, 2023; the employer allowed his employees to work from an aerial lift without first ensuring his employees were trained on the hazards associated with falls to include but not limited to inspecting fall protection and the requirement to use fall protection in an aerial lift  exposing his employees to a fall hazard of 30 feet  to the ground below.
