29 CFR 1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about March 30, 2023 employees were exposed to injury from being struck by falling objects while working on lower ground level beneath an occupied scaffold.

29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. end frames were used as a means of access. On or about March 30, 2023 employees were exposed to fall injury while working from 2nd and 3rd level working platforms of a tubular welded scaffold without safe ladder access.
