Willow Springs Recovery located at 1128 TX-21, Bastrop, TX 78602:  On or about March 31, 2023, and at times prior thereto,  the employer had not provided employees permitted to voluntarily wear N95 filtering facepiece respirators with the information contained in Appendix D to 29 CFR 1910.134, exposing employees to respiratory hazards.
