29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a) Location - 2 Maple St, Merrimac, MA :  On or about 4/3/2023, the competent person(s) onsite did not address construction hazards, such as but not limited to, fall hazards.   An employee was exposed to a fall hazard of approximately 14-feet to a lower level without any fall protection system in place.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) Location - 2 Mapleton St., Merrimac, MA:  On or about 4/3/2023, the employer did not ensure that an employee who was performing leading edge work on the 2nd floor of a new residential home were protected from falls of 6-feet or greater to the next lower level.

This condition exposed an employee who fell approximately 14-feet to the ground.
