29 CFR  1910.28(b)(13)(i): 29 CFR 1910.28(b)(13)(i): The employer did not ensure that when work is performed less than 6 feet (1.6 m) from the roof edge, each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system.  On or about April 3, 2023, at the establishment located at 6151 Highway 51 Hazelhurst, Wisconsin; employees were exposed to fall hazards greater than 21 feet while performing snow removal activities on the roof of the production area.

29 CFR  1910.132(d)(2):The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  On or about April 3, 2023, at the establishment located at 6151 Highway 51 Hazelhurst, Wisconsin; the employer could not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.
