29 CFR 1910.303(b)(6): Equipment located in damp or wet locations was not identified for that purpose. (29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures. Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations shall be installed so as to prevent moisture or water from entering and accumulating within the enclosures and shall be mounted so there is at least 6.35-mm (0.25-in.) airspace between the enclosure and the wall or other supporting surface. However, nonmetallic enclosures may be installed without the airspace on a concrete, masonry, tile, or similar surface. The enclosures shall be weatherproof in wet locations.) (a) On or about April 5, 2022, the electrical service on the northeast side of the facility was exposed to water infiltration from a leaking roof. This exposed employees to an electrical shock hazard.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) On or about April 5, 2023, employees engaged in painting, were exposed to hazardous materials, such as but not limited to, paints and solvents. The employer had not ensured each employee utilizing chemicals were provided with effective training on the hazard communication program, Safety Data Sheets, and how to access the Safety Data Sheets.
