29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying objects while using a nail gun.  On or about April 5, 2023, and at times prior thereto, at the residential construction site, on the right elevation of the two-story home, where employees operate pneumatic nail guns to install cornice did not wear or have safety glasses to protect the eyes and face from flying objects, exposing the employee to struck-by injuries.

On or about April 5, 2023, and at times prior thereto, at the residential construction site, the right elevation of the two-story home, the employer did not ensure while employees work from the pump jack were protected from fall hazards, where an employee working at a height of 13 feet above a lower level without fall protection was exposed to fall hazards and injuries.

On or about April 5, 2023, and at times prior thereto, at the residential construction site, the right elevation of the two-story home, the employer did not ensure that the employee had fall protection to protect from falling at 13 feet above a lower level while standing on a plank of the pump jack scaffold, exposing the employee to fall injuries.

On or about April 5, 2023, and at times prior thereto, at the residential construction site, the right elevation of the two-story home, the employer did not ensure that the employee was trained by a qualified person to recognize and minimize scaffold hazards, while installing the cornice and standing on a plank of the pump jack scaffold at 13 feet above a lower level, exposing the employee to fall injuries.
