29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  On April 5, 2023, and times prior thereto, the employer did not ensure fire extinguishers were identified with a fire extinguisher sign and ready accessible for employees.  Employees were exposed to fire and smoke hazards.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.   On April 5, 2023, and times prior thereto, extension cords, wrapped around metal conduit, was used as permanent wiring to energize equipment on the production line exposing employees to electrical shock hazards.  a) At line 6, extension cord wrapped around metal conduit and plugged into the line 6 machine, b) At line number 1, extension cord wraped around conduit plugged into the water pump placed inside 55 gallone drum.
