Roof area of the residential job site located at 3155 Garden Brook Road, Jacksonville, Florida 32208: On or about April 7, 2023, the employer exposed employees to eye injuries caused by air-born debris and flying objects, in that eye protection was not utilized by the employees, while tearing off old roof shingles and while installing sheathing using pneumatic nail guns.

At a residential job site located at 3155 Garden Brook Road, Jacksonville, Florida: On or about April 7, 2023, the employer exposed five employees to fall hazards ranging from 9 feet up 25-foot, in that fall protection was not utilized by employees while installing roof shingles.

At a temporary residential worksite, located at 3155 Garden Brook Road, Jacksonville, Florida 32208; On or about April 7, 2023, the employer exposed an employee to  head injuries, in that hard hats were not utilized by employee while picking up debris from around the home.
