29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee was using appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On 10 April 2023, one employee of MF Carpentry Inc. was working on the roof of a single-family home located at 177 Rhythm Point Drive in Saint Peters, Missouri. He was not utilizing eye protection while using a pneumatic nail gun.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were an employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On 10 May 2023, two employees under MF Carpentry Inc were working on a "5 pitch" residential roof located at 177 Rhythm Point Dr in Saint Peters, Missouri, 63376. They were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing them to falls to the ground below.
