229 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  On April 10, 2023, employees of T. Rife Exteriors were installing exterior siding onto the south side of a residential building located at 115 Liberty Valley Drive in Foristell, Missouri.  The employer did not ensure employees working on a ladder jack scaffold approximately 24 feet high were protected from falling to the lower level.  The employees were exposed to fall hazards when not utilizing personal fall arrest systems.

29 CFR 1926.452(k)(1): Platforms on ladder jack scaffolds exceeded 20 feet in height:  On April 10, 2023, employees of T. Rife Exteriors were installing exterior siding onto the south side of a residential building located at 115 Liberty Valley Drive in Foristell, Missouri.  The employer did not ensure the scaffold height did not exceed 20 feet in height.  The ladder jack scaffold platform was measured to be approximately 24 feet high.  The employee working on the ladder jack was exposed to fall hazards.
