1926.503(a)(2)(iv)  The employer did not ensure that each employee was trained, as necessary by a competent person in the role of each employee in the safety monitoring system where the system was used: 29 CFR  1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function:   (a) On or about April 13, 2023, the employee trained and designated as the safety monitor was engaged in activities that could distract that monitor from the primary function of  observing, or monitoring all exposed employees.  The employee designated as the safety monitor was unable to watch all exposed employees working on a roof awning in the same line of sight due to location of the monitor and the exposed employees working on opposite sides of the awning.  This exposed employee(s) to a fall hazard of 12.8 feet onto concrete below.
