29 CFR  1926.453(b)(2)(v):A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  On or about April 14, 2023, and times prior thereto, at a multi-residential dwelling unit #13, an employee worked inside an aerial lift basket without a lanyard to attach to the basket and or boom, exposing the employee to a fall 6 foot or greater to a lower level.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about April 14, 2023, and times prior thereto, at the multi-residential dwelling unit #13, the employer failed to ensure an employee working at a height of 6 feet or more without fall protection was provided prior training in recognizing fall hazards to prevent and minimize the hazard.
