On or about April 14, 2023, employees were installing a metal sign on top of commercial store roof above other employees working on the ground without hard hats, exposing employees to the hazard of being struck-by falling objects.

On or about April 14, 2023, an employee was utilizing an aerial lift without the use of a personal restraint system at heights above approximately 10 feet, exposing employees to fall hazards.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.   On or about April 14, 2023, employees were installing signage on the roof of a business at heights above 10 feet without fall protection, exposing employees to the fall hazards.
