29 CFR 1910.147(a)(2)(ii)(B):29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into area on a machine or piece of equipment's point of operation. Location: Fish processing floor- Pier Fish 68 Conway Street, New Bedford, MA On or about February 9, 2023, an employee was attempting to clear a jam in the coffin scaler machine when his hand entered the point of operation causing a hand injury.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical Location: Pier Fish Co, Inc.- 68 Conway Street, New Bedford On or about April 17, 2023, a partially filled uncovered 5-gallon bucket (approximately 2/3 filled) in a "LemonSuds" bucket of a dark blue liquid was discovered in the chemical closet which management could not identify the contents. Additional research determined the dark blue liquid was not LemonSuds" as the SDS for "LemonSuds' indicates it is a yellow liquid. It is likely the blue liquid is AP ProKleen as the SDS for that product lists its appearance as a clear blue liquid and none of the other products in the cleaning room are blue per their SDS's.
