29 CFR 1926.1053(b)(9):The area around the top and bottom of ladders was not kept clear:  a) On or about April 18, 2023, employees were exposed to fall and puncture wound hazards where a load of roof tear-off material was accumulated at the bottom of a portable extension ladder used to climb to and from the roof deck.    No abatement certification required for this violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) On or about April 18, 2023, employees were exposed to the hazard of falling approximately 20 feet from a pitched roof while working without fall protection.   NV Roofing, Inc., dba New Vision Home Restoration was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1544597, citation 1, item 1, and was affirmed as a final order on December 2, 2021, with respect to a worksite located at 10933 Fawn Drive, Orland Park, Illinois.    No abatement certification or documentation required for this violation.
