29 CFR  1917.12: The employer did not eliminate, to the extent possible, conditions causing slippery working and walking surfaces in work areas used by employees.   a)  Port of Whittier Terminal- timber trestle section that spans between Cell No. 3 and the East Pass-Pass Dock:  On or about March 28, 2023 and at times prior thereto,  employers did not remove to snow and or ice on the surface of the trestle and longshore workers had to use it to access winch house, exposing employees to slip, trip and fall hazards.

29 CFR 1917.112(b)(1):  Guardrails were not provided at locations where employees were exposed to floor or wall openings or waterside edges, including bridges or gangway-like structures leading to pilings or vessel mooring or berthing installations, which presented a hazard of falling more than 4 feet or into the water:   a) Whittier Terminal Dock - Timber trestle section that spans between Cell No. 3 and the East Pass-Pass Dock: On or about March 28, 2023 and at times prior thereto, the trestle at waterside edge was unguarded, exposing longshore employees to fall hazards to the rocks and water below.

29 CFR  1917.118(e)(1):  Fixed ladders more than 20 feet (6.1 m) in height shall be provided with a cage, well, or ladder safety device.  a)  Port of Whittier:  On or about March 28, 2023 and at times prior thereto, on the  A-Frame Lift Stand, longshore employees accessed a fixed ladder to get to the top and grease the winch without fall protection, exposing them to fall hazards of approximately 20 feet.
