29 CFR  1926.405(g)(2)(iii):Flexible cords were not used in continuous lengths without splice or tap:  On or about April 19, 2023 the employer Regent Electric Inc., at worksite located at 15 S. Ontario St. Toledo OH 43604, failed to ensure the flexible cord energizing a temporary lighting on the second floor of the structure, was of continuous length and insulated appropriately. The temporary lightings flexible cord maintained a splice which was partially wrapped with electrical tape. Employees working in the vicinity of the low hanging flexible cord (58" from the walking/working surface) in this energized 120 volt condition were exposed to electrical shock.
