29 CFR  1926.405(g)(2)(iii):Flexible cords were not used in continuous lengths without splice or tap:  On or about April 19, 2023 the employer Precision Epoxy Flooring LLC, at worksite located at 15 S. Ontario St. Toledo OH 43604, failed to ensure employees working on the second floor were protected from electrical shock hazards. The temporary lightings flexible cord maintained a splice which was partially wrapped with electrical tape. Employees working in the vicinity of the low hanging flexible cord (58" from the walking/working surface) in this energized 120 volt condition were exposed to electrical shock.
