29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) On or about April 20, 2023 at the location of 1223 Nicholson Ave., Lakewood, OH: The employer did not ensure that employees performing roofing activities on a steep slope roof were protected from fall hazards. Employees were exposed to fall hazards of 8 feet and 18 feet to the ground below while performing roof tear off and shingle replacement.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about April 20, 2023 employees were exposed to a fall hazard of approximately 8 feet while performing roofing activities on a residential home located at 1223 Nicholson Ave., Lakewood, OH . Employees were using an extension ladder to access the roof and the employer did not ensure they extended at least 3 feet above the upper landing surface.
