29 CFR  1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.  a) Pottsgrove Glass Co., Inc., - Perth Amboy High School Construction Site on Convery BLVD. and Dorothy Ave., Perth Amboy NJ 08861;  On April 05, 2023, employees engaged in hoisting steel/aluminum window channel curtains into place by use of a JLG Ultra Boom Lift with a SkyGlazier attachment by tying a sling to channels and the attachment, which is an unauthorized use of lift by the manufacturer. In doing so employyes were exposed a tip over hazard of  the lift, damage to equipment that placed workers in basket at risk of injury.

29 CFR  1926.453(a)(2):Aerial lifts were "field modified" for uses other than those authorized by the manufacturer provided and the modified use had not been certified in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969 and this section and to be at least as safe as the equipment was before modification.  a) Pottsgrove Glass Co. Inc. - Perth Amboy High School Construction Site on Convery BLVD. and Dorothy Ave., Perth Amboy NJ 08861;   On 05 April 2023,  employee operating a JLG Ultra Boom Lift 1250 APJ with SkyGlazier tray attachment used the lift to carry personnel and to hoist 260 pound window curtain wall frame channels above by tying  them to the SkyGlazier attachment without prior written authorization by the manufacturer.  The equipment was modified for unauthorized use of the lift and attachment,  as the manual specifically states not to be used to hoist in a crane type manner.
