29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. a) On or about April 19, 2023, the employer did not ensure that an electrical outlet located in the shipping department was not damaged or broken. As a result, employees were exposed to electrical shock hazards if the exposed wires were to energize the conduit. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. a) On or about April 19. 2023, the employer had a Masterplat Robopac (MN: PGS Plus SN: 30227915) automatic stretch wrapper plugged into a relocatable power tap. Relocatable power taps are not designed to be used with industrial equipment, exposing employees to electrical and fire hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
