29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.   Location: 108 Main St. CARVER, MA  02330  On or about April 12th, 2023, employees were exposed to falls from heights up to twenty feet to the ground while working on a pump jack scaffold system where a competent person was not present during the erection of the scaffold nor did a competent person inspect the scaffold system prior to allowing employees to work on the scaffold.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   Location: 108 Main St. CARVER, MA  02330  On or about April 12th, 2023, employees were exposed to falls from heights up to twenty feet to the ground while accessing a Pump Jack scaffold system where the portable ladder used to access the upper landing surface did not extend at least 3 feet above the landing surface.
