On the roof of a temporary residential worksite, located at 78 Naugatuck Drive - Lot 2B, Jacksonville, FL 32225: On or about April 26, 2023, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install roof shingles.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a.  On a 3:12 pitch roof of a temporary residential worksite, located at 78 Naugatuck Drive ? Lot 2B, Jacksonville, FL 32225: On or about April 26, 2023, the employer exposed employees to fall hazards, in that, fall protection was not utilized by employees while performing roofing activities and were being exposed to fall hazards ranging from 11 feet to 17 feet fall hazards.
