29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use:  (a) 37 Hickory Corner Road Philippi, WV 26416: On April 21, 2023, an employee was working from a pallet platform raised approximately 16ft from ground level with a Variable Reach Forklift. The Variable Reach Forklift was not designed by the manufacture for this intended use, exposing the employee to fall hazards.

The employee was not protected from falls by the use of personal fall arrest systems or guardrail systems.
