On or about May 19, 2022, employees performed masonry tasks on a scaffold that a competent person did not inspect before allowing employees to work on the scaffold.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. For falling objects too large, heavy, or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall.  On or about April 25, 2023, on-site employees performed masonry work on a scaffold with no toe boards. Employees were not protected from falling objects on the scaffold's upper platforms.

On or about May 19, 2022, employees performed masonry tasks on a scaffold where the space between the platform, uprights, and supporting planks was more than 1 inch (2.5 cm) wide.  DRA Enterprises, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number #1378778, citation number #1, item number #1-a and was affirmed as a final order on 06-24-2019, with respect to a workplace located at 2680 Bogan Walk, BATON ROUGE, LA 70802.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffolds, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access.  On or about May 19, 2022, employees were performing masonry tasks on a scaffold; the employer did not provide a ladder for the employees to access the upper levels of the scaffold.  DRA Enterprises, LLC was previously cited for a violation of this occupational safety and health standard, or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number #1378778, citation number #1, item number #1-C and was affirmed as a final order on 06-24-2019, with respect to a workplace located at 2680 Bogan Walk, Baton Rouge, LA 70802.
