29 CFR  1904.41(a)(1):29 CFR 1904.41(a)(1) If your establishment had 250 or more employees at any time during the previous calendar year, and this part requires your establishment to keep records, then you must electronically submit information from the three recordkeeping forms that you keep under this part (OSHA Form 300A Summary of Work-Related Injuries and Illnesses, OSHA Form 300 Log of Work-Related Injuries and Illnesses, and OSHA Form 301 Injury and Illness Incident Report) to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms.  Blanchard Valley Health System doing business as Blanchard Valley Regional Health Center located at 1900 South Main Street in Findlay, Ohio:   On or about March 2, 2023 the employer failed to electronically submit the company's OSHA Form 300A Summary of work related injuries and illnesses for 2022 calendar year to OSHA's Injury Tracking Application (ITA). The employer had an average of 1930 employees at the Findlay, Ohio facility in 2022 requiring them to submit the OSHA 300 A information to OSHA by March 2, 2023.
