29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At the jobsite located at 821 N. 2nd St. Salina, KS, an employee engaged in second floor deck construction activities was exposed to fall hazards of approximately 8 feet from the bottom of the wooden frame support of the deck frame to the concrete floor below in that fall protection was not utilized.

Employees were utilizing 3 Werner A-frame ladders with wooden planks as a makeshift scaffold against manufacturer's design/purpose.
